Insurance

We accept most insurance carriers. Please have your card with you at the time of your visit. If you have a commercial insurance, you are responsible for a minimum of $55.00 per visit. Any remaining balance once the claim is processed, will be due upon receipt of invoice. If we are not an in-network provider for your insurance, please contact your member services and request a single case agreement.

We accept all types of Georgia Medicaid including all CMO's.
